Short Summary:
The Expanded School Mental Health (ESMH) Care Coordinator provides school-based care coordination, supportive counseling, and targeted case management services within assigned schools, supporting students in the Wood County school system.
Responsibilities:
- Plans, implements, and supports programs to reduce school-related concerns.
- Maintains a visible presence throughout assigned school buildings and grounds.
- Collaborates with school and community partners.
- Coordinates program-related staff development for faculty and staff.
- Observes and documents student behavioral concerns.
- Provides classroom presentations on program-related topics.
- Communicates effectively with students, parents, and community partners.
- Serves on the Student Services Team and School Crisis Team.
- Completes monthly reports and other required accountability reports.
- Travels between schools and the office as needed.
Requirement:
- Bachelor’s degree in a human services field or equivalent experience.
- Valid driver’s license and automobile insurance.
- Ability to communicate clearly and work effectively as part of a team.
- Basic math skills are required.
