Short Summary:
The Registered Nurse Navigator is responsible for providing comprehensive care coordination and case management services to patients with complex medical needs. This role includes creating relationships with discharge planners and the patient’s healthcare team, assessing needs, developing care plans, and advocating for patients' rights.

Responsibilities:
- Identify and qualify potential clients.
- Assess patient needs and develop individualized care plans.
- Cultivate and maintain strong relationships with referral sources.
- Coordinate care with other providers.
- Advocate for patients' rights and interests.
- Educate patients and families about health conditions and treatment options.
- Monitor patient progress and adjust care plans as needed.
- Manage patient records and documentation.
- Participate in quality improvement initiatives.
- Provide clinical expertise with sales representatives.
- Conduct presentations and site tours for prospective clients.

Requirement:
- Bachelor's Degree (BSN) from an accredited nursing program.
- Minimum of 2 years of experience as an RN.
- Pediatric experience required.
- Current Texas RN license and BLS Certification.
- Bilingual in English and Spanish is required.
