Short Summary:
The Clinical Pharmacist Specialist role is to provide evidence-based collaborative patient-centered care to Norton Healthcare patients, optimizing medication therapy and developing system-wide strategies for desired clinical outcomes.
Responsibilities:
- Optimize medication therapy by developing programs/guidelines that improve practice and patient outcomes.
- Provide efficient, cost-effective patient-centered care.
- Offer verbal and written consultations concerning medication therapy daily.
- Participate in the implementation and maintenance of guidelines and order sets.
- Educate pharmacy residents, students, physicians, nurses, and other healthcare team members.
- Detect, monitor, and report adverse drug events.
- Support the QI process through ongoing development and monitoring of performance improvement indicators.
- Assist in the effective use of resources and ensure budget targets are met.
Requirement:
- Requires three years of pharmacy experience or completion of an accredited PGY-1 residency or fellowship.
- Bachelor’s degree and pharmacist licensure.
- BCPS preferred.
