Responsibilities:
- Intervening in crisis situations
- Maintaining supportive relationships with residents and families
- Developing individualized psychosocial resident plans of care
- Assisting residents in meeting goals in their psychosocial plans of care
- Observing, documenting, and reporting resident behavioral/psychosocial conditions
- Documenting accurate behavioral and psychosocial information in progress notes
- Reviewing resident advance directives
- Completing psychosocial assessments/reviews
- Providing education and resources regarding advance directives and resident rights
- Assisting in making referrals and arranging appointments for ancillary care services
Requirement:
- Current licensed social worker or social service designee certification in facility state
- Strong interpersonal, documentation, and assessment skills
- Basic computer skills
- One year long-term care experience preferred
