Responsibilities:
- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions.
- Take patient history, examine, determine diagnoses, and provide written documentation of patient visits.
- Communicate with patients, caregivers, agency nurses, and other providers to assure proper diagnosis.
- Maintain patient confidentiality and participate in professional development activities.
Requirement:
- Active unrestricted NP license.
- Board certification (ANCC/AANP/NCCPA), BLS.
- Current enrollment in Medicare/Medicaid.
- Valid driver’s license and good driving record.
- Outstanding EHR skills.
- Geriatric experience preferred.
