Short Summary:
The Community Outreach Team promotes Foodsmart's foodcare program in partnership with Santa Clara Family Health Plan (SCFHP). This role focuses on increasing enrollment by identifying community partners and events, and creating strategies for onsite activations and referrals.

Responsibilities:
- Identify active Community-Based Organizations (CBOs), Faith-Based Organizations (FBOs), and Federally Qualified Health Centers (FQHCs) serving SCFHP members.
- Evaluate local community events and health fairs for Foodsmart presence.
- Attend enrollment events to raise awareness of Foodsmart's program and SCFHP benefits.
- Cultivate relationships with organizations to build awareness of Foodsmart.
- Work with the marketing team to create event materials in various languages.
- Provide education on healthy eating and SNAP/CalFresh benefits.
- Screen event attendees for Foodsmart eligibility and schedule nutrition coaching appointments.
- Maintain a comprehensive activity log of events, referrals, and enrollment outcomes.
- Attend bi-weekly team meetings and support the CHC referral activation strategy.

Requirement:
- Bilingual in English and Spanish (required).
- Strong organizational skills and experience with Medi-Cal populations or community health programs preferred.
- Familiarity with the Santa Clara County health ecosystem is highly valued.
