Short Summary:
Accompany Health is on a mission to provide dignified, high-quality care to patients with complex needs. As a Health Advocate, Float, you will ensure continuity of care across patient panels, stepping in when regular team members are unavailable.
Responsibilities:
- Act as a flexible resource to cover critical parts of the business, completing in-home and virtual visits.
- Independently capture vitals using connected devices and digital tools.
- Identify and follow up on critical social needs to ensure continuity in resource navigation.
- Support essential care coordination needs, including medication changes and appointment scheduling.
- Facilitate virtual visits and troubleshoot technology for patients and clinicians.
- Review and understand patient care plans to coach patients effectively.
- Maintain accurate EHR documentation for seamless handoffs back to the primary care team.
Requirement:
- Experience in a direct patient care, home-based role, or float pool setting.
- Strong interpersonal and communication skills.
- Competence in basic clinical tasks and technology.
- Valid unrestricted driver's license and access to an insured vehicle.
- Preferred: Medical Assistant (CMA) or Community Health Worker (CHW) certification, or 2+ years experience in a healthcare setting.
