Short Summary:
The Community Care Advocate connects employees to community and company resources to meet social determinants of health and needs of the employee, serving a Fortune 500 company in the retail industry.
Responsibilities:
- Identify and make referrals for work-life, financial & legal, and basic needs support.
- Link employees to appropriate local and national resources via one-on-one consultation and referral lists.
- Work with customer internal care teams on behalf of employees in need of emergency funding and resource referrals.
- Offer consultation related to navigating mental health resources and available workplace benefits.
- Assist in linking employees to appropriate mental health treatment options and provide support throughout applications and follow-ups.
- Document and track metrics/outcomes related to social work involvement and referrals.
- Represent Spring Health and support program parameters and goals.
- Communicate with local points of contact and submit reporting documentation of all client and organizational activities.
- Proactively develop relationships with key customer contacts and promote the community care advocate service offering to employees.
Requirement:
- Bachelor’s degree in a related field.
- English and Spanish fluency required.
- Experience working with diverse populations within the social work industry.
- Ability to balance the needs of multiple stakeholders.
- Excellent communication skills and tech-savvy.
- Experience connecting individuals to resources quickly in times of crisis is a plus.
