The Emergency Bed Program Case Manager provides trauma-informed, survivor-centered case management to support adults entering Samaritan Village's Emergency Bed Program. This role coordinates emergency admissions, conducts comprehensive needs assessments, develops individualized service plans, and connects participants with essential community resources to promote safety, stabilization, and recovery. The ideal candidate is compassionate, organized, and experienced in crisis intervention, case management, and working with vulnerable populations in a fast-paced, collaborative environment.

Description

The Emergency Bed Program Case Manager helps to screen referrals, coordinate emergency admissions, and welcome new participants into Samaritan Village's Emergency Bed Program, ensuring immediate safety, stabilization, and access to essential services. This position conducts comprehensive needs assessments, develops individualized stabilization and service plans, and collaborates with community agencies, healthcare providers, victim service organizations, businesses, faith-based organizations, and other community partners to coordinate participant care.

This position provides trauma-informed, survivor-centered case management through crisis intervention, emergency resource coordination, advocacy, safety planning, and rapid service linkage while supporting participants as they transition from emergency shelter into longer-term housing, restorative programming, or other appropriate community placements. The Case Manager maintains timely and accurate documentation, promotes participant empowerment and recovery, and upholds the security and confidentiality required for this specialized program.

DUTIES

  • Screen participant referrals and coordinate emergency admissions based on program eligibility and participant safety needs.
  • Coordinate timely access to medical care, behavioral health services, victim advocacy, legal resources, and other emergency support services based on participant needs.
  • Conduct comprehensive assessments and develop individualized stabilization and service plans that address participants' immediate safety needs, goals, and ongoing recovery.
  • Monitor participant progress, reassess needs, and modify stabilization and service plans to promote successful outcomes and transitions.
  • Collaborate with participants to review shopping lists and arrange bi-weekly shopping for common groceries and household/cleaning supplies.
  • Coordinate emergency and community-based services including healthcare, behavioral health, legal assistance, financial assistance, transportation, education, employment, and other supportive services.
  • Provide relevant referrals to agencies and service providers based on the goals and objectives mutually agreed upon with the resident/participant.
  • Build and maintain collaborative relationships with community partners to ensure participants have timely access to essential resources and supportive services.
  • Collaborate with healthcare providers and service providers to facilitate care coordination.
  • Develop individualized transition and a personalized discharge plan for each participant.
  • Coordinate participant appointments, transportation, and service schedules to ensure timely access to required services.
  • Assist with providing life-skills and survivor care curricula
  • Maintain accurate and up-to-date case records while maintaining confidentiality.
  • Documentation of reports must be timely, accurate and objective as required by agency standards and program expectations.
  • Actively participate in staff meetings and trainings
  • Rotate on-call duties as scheduled.
  • Perform other related duties as required.

ESSENTIAL SKILLS

  • Skilled in crisis management, conflict resolution, and de-escalation techniques (preferred).
  • Self-starter shows initiative for effective work without direct supervision (administration and organization)
  • Effective communication and collaboration skills
  • Administrative and computer proficiency, including use of search engines, email, Google Workspace, and Microsoft Office Suite.
  • Able to meet face-to-face with individuals of diverse backgrounds and disabilities, including physical, mental, substance abuse, HIV/AIDS, etc., and able to develop rapport, provide information, and refer with sensitivity to cultural issues
  • Ability to be proactive and detail-oriented
  • Ability to lift, stoop, bend, reach, and move about freely

QUALIFICATIONS

  • Bachelor's degree in a human services-related field and a minimum of two (2) years of experience in human trafficking services or a related social services setting; OR
  • A minimum of four (4) years of progressive administrative or management experience, combined with at least two (2) years of experience in human trafficking services or a related social services setting.
  • Experience with anti-trafficking programs, trauma informed case management, crisis stabilization/intervention, emergency services (preferred)
  • Possess reliable transportation
  • Schedule availability to support a 24/7 emergency shelter operations/ environment. Flexibility among shifts may be required from time to time (holidays, weekends, and on call)
  • Must pass criminal background checks
  • Possess a valid driver’s license and proper insurance
  • Must be able to operate a six-passenger vehicle.
  • CPR/First Aid Certification (preferred) or obtained within required

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
