Work Arrangement:
Services delivered at Habitat PACE Center Sacramento and participant homes.
Short Summary:
The Physical Therapist- Part Time provides physical therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and educating participants and caregivers on exercise programs and DME use.
Responsibilities:
- Provides physical therapy evaluation and treatment on an acute, restorative, and maintenance level to program participants.
- Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
- Assesses condition, creates a treatment plan, and discusses implementation of therapy with participants and caregivers.
- Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart.
- Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
- Maintains the quality assurance, safety, environmental, and infection control programs.
Requirement:
- Bachelor’s Degree in physical therapy.
- Completion of a physical therapy program accredited by the APTA or equivalent.
- Active Physical Therapist license issued by California.
- A minimum of one year experience working with the frail or elderly.
- Current CPR/BLS certification.
- Ability to thrive in a fast-paced, evolving environment.
