Short Summary:
The Outreach & Enrollment Supervisor leads Habitat Health's Outreach and Enrollment team, driving participant growth through strategic leadership, operational excellence, and talent development. This role oversees the end-to-end outreach, intake, and enrollment functions, ensuring a seamless, participant-centered experience.
Responsibilities:
- Leads, coaches, and develops a high-performing Outreach and Enrollment team.
- Develops and executes outreach and enrollment strategies.
- Monitors team performance and enrollment outcomes.
- Partners with center leadership for a coordinated enrollment experience.
- Provides leadership on complex enrollment cases.
- Builds and maintains strategic relationships with healthcare providers and community organizations.
- Leads community outreach initiatives and educational events.
- Ensures compliance with regulatory requirements.
- Drives continuous improvement in participant experience and operational efficiency.
Requirement:
- Bachelor's degree in a related field or equivalent experience.
- Minimum of three years in healthcare sales, outreach, or related fields.
- Minimum of one year of experience leading teams.
- Experience working with older adults or within PACE, Medicare, or Medicaid environments.
- Strong leadership, coaching, and talent development skills.
- Strong communication and relationship-building skills.
- Ability to analyze operational data for decision-making.
- Experience with Microsoft Office and CRM systems.
- Valid driver's license and reliable transportation.
